Exocrine Pancreatic Conditions
Exocrine pancreatic insufficiency (EPI) occurs in people whose pancreas does not produce enough digestive enzymes to break down foods and absorb nutrients. EPI is often caused by conditions such as chronic pancreatitis (inflammation and damage to the pancreas over time) and cystic fibrosis (a genetic condition that affects the lungs, pancreas and other organs).
Patients with this disorder commonly have trouble digesting fats, which causes abdominal pain, gas, bloating, diarrhea, fatigue, fatty stools and weight loss or a failure to thrive in children. If a doctor suspects EPI, they may order a fecal elastase test (FE-1), fecal fat test or a secretin pancreatic function test.
Treatment typically includes pancreatic enzyme replacement therapy (PERT), a high-calorie, high-fat diet and vitamins. Patients whose EPI is caused by chronic pancreatitis may also require pain management, surgery and lifestyle changes such as dietary adjustments or avoiding smoking and alcohol. EPI is a lifelong disease, and treatments focus on ensuring the patient gets the nutrients their body needs to maintain health.
